Payroll Processing Forms
The forms available on this
page allow you to initiate several functions associated with payroll
processing. There are four options from which to choose.
Option A - You can obtain a paper copy of the form, manually
fill out the form, and fax the completed for to us. To request
a paper copy of the form(s) contact us.
Option B - You can download an electronic copy of
the form, print it to make a master copy, and then fill out the
forms manually and fax them to us.
The master forms are in the Adobe "pdf" file format. They
require Adobe Acrobat Reader. If you do not have Acrobat Reader
installed on your computer you can download a free copy by clicking
on the Acrobat Reader icon.
Option C - You can fill out a copy on your computer.
To do this, click on the appropriate form in the following list.
Note: These forms require that you have Microsoft Word version
98 or higher installed. The forms indicated by * require that
you allow macros to execute within Word.
